Case Manager / Home Health Care

Home Helpers is seeking a Licensed Social Worker, LPN, PT or similar background with home health care experience for the position of Case Manager/ Care Coordinator. This position will be approx 30+ hrs/wk hourly to start during training and will quickly develop into full time salary + bonuses for the right person. Mileage stipend,company cell phone and navagation unit provided.


Primary responsibilities include:

Assessing the needs of potential clients in their homes Creating care plans for clients Following up with clients to ensure optimum care Management of caregiver staff Presentations and in-service meetings Identify community resources for clients and business growth

Qualifications
The chosen candidate must possess:

A current Social Worker license,LPN,PT or similar field with case management experience.
Experience in understanding the needs of Seniors Computer proficiency (Word, Excel, Powerpoint, etc.) Scheduling experience a plus.
Strong communication skills, oral and written.
Excellent organizational and follow up skills
